in the skin or mucous membranes caused by the human papilloma virus. Warts are also known as verrucae or condylomata. Warts
are very common - sometimes warts on the hands, verruca vulgaris, plantar warts and plane warts resolve without treatment
but the other times they do not. Warts can be found in individuals of any age and in any location. They are particularly
common in children and teenagers. Generally warts occur in those who are sexually active. There is a balance between the virus
and the individual’s immune system. Situations that reduce immunity will make it easier for the warts to spread and
become more susceptible. There are more than 50 different types of HPV Symptoms .These are DNA viruses. The different types are given numbers. It appears that each number either gives a specific type
of wart or a specific location. There is an uncommon type of human papilloma virus that can have a potential to develop cancers
especially in the cervix. These are known as Type 16 and 18. Warts are transmitted by skin to skin contact, although plantar
warts may well be transferred from moist floors. The transmission of warts depends in part on the immunity and susceptibility
of the individual involved. Warts have an incubation period of many weeks to many months. Types of Warts: Verruca Vulgaris (the common wart) These are thickened bumps called
papules or plaques. They have a rough surface and frequently will have small tiny spots which are from blood vessels that
have clotted. They are most commonly seen on the hands, elbows and knees. Flat Warts se are flat-topped,
smoother looking warts. They are brown or skin coloured. They are most commonly seen on the face of women. They are spread
easily by shaving. Plantar Warts are seen on the under-surface of the feet. They are thickened. They
can be confused with corns or calluses. Occasionally they can be quite widespread involving large surfaces. They tend to grow
into the skin rather than coming outwards as most other warts.Genital Warts appear in the genital peri-anal
skin. . They can look like common warts or sometimes they are skin coloured, small papules. They vary in size from tiny papules
to much larger typically warty growths. Occasionally they are pigmented. There is a subgroup of these warts caused by HPV
16 and 18 which have been named bowenoid papulosis. They have a small chance of becoming cancerous. Some of the genital warts
have been associated with carcinoma of the cervix, Bowenoid papulosis and Bowen's disease.Warts are usually obvious to
the experienced doctor. A biopsy can confirm the diagnosis although it is not commonly required. There are specific tests
to type the human papilloma virus although these are not done routinely. NYC Dermatology Board Certified Dermatologist
NY Dermatology takes care of all these condtions. NON SURGICAL FACELIFT - THE THREADLIFT Good candidates for ThreadLift are those patients who
are starting to see some droopiness of the brow, cheeks, jowls and/or neck and would like to see a lift effect, but do not
want the risk, scars, or recovery of a real browlift, facelift, or necklift operation. The ThreadLift procedure can achieve
60-70% of what a real invasive surgical procedure can deliver. Good candidates must have realistic expectations and be prepared
to follow the postoperative recovery protocol and must be accepting of the risks. Poor candidates include those patients with unrealistic expectations, have
uncontrolled medical illnesses that affect would healing, are grossly overweight (with heavy faces) or are excessively thin,
loose skin. The ThreadLift may be combined with other, more minor surgical procedures, such as blepharoplasty (lid lift procedures),
liposuction, microfat transplantation, Botox, Injectable Fillers, Thermalift, Thermage, Fraxel skin resurfacing, or FotoLift-Polaris

Vitiligo<dermatology> This disorderis characterised by regionsof otherwise normalskinof nonpigmented whitepatches of varied sizes, often symmetrically distributed and usually bordered by hyperpigmentedareas. Hairin the affectedareas is usually, but not always, white. Keloid scars :Occasionally scars enlarge spontaneously to form
firm, smooth, hard growths called keloids. Keloids may be uncomfortable or itchy, and may be much larger than the original
wound. They can arise soon after the injury, or develop months' later. It is not known why keloids appear. While most
people never form keloids, others develop them after minor injuries, even insect bites or pimples. Keloids may form on any
part of the body, although the upper chest and shoulders are especially prone to them. Dark skinned people form keloids more
easily than Caucasians. Keloid scars are usually just a cosmetic problem. They never become malignant. As wounds heal, scar
tissue forms, which at first is often red and somewhat prominent. Over several months, a scar usually becomes flat and pale.
If there is a lot of tension on a healing wound, the healing area is rather thicker than usual. This is known as a hypertrophic
scar.Hypertrophic scars generally settle in time but keloids may prove resistant to treatment. The following measures are
helpful. Dressings should be worn for 12 to 24 hours per day, for at least 8 to 12 weeks, and perhaps for much longer. Self-adhesive
polyurethane scar reduction patches Silicone gel dressings Pressure dressings Surgical excision (but may result in a second
keloid even larger than the original one) Corticosteroid injection, repeated every few weeks Cryotherapy. Superficial X-ray

Pseudfolliculitis barbae ( Razor bumps) dermatology> Erythematous follicular papules or, less commonly, pustules resulting from close shaving of very curly hair. The growing tips of hairs consequently reenter the skin adjacent to the follicle producing ingrown hairs. Pseudofolliculitis of the beard area is very common in blacks This is especially problematic for men of African descent and other people with curly hair. If left untreated over time, this can cause keloid scarring in the beard area. http://www.dermatologistsnyc.com Hives (Urticaria) are pink swellings that can vary in size from as small as a pin to very
large erythematous (red) plaques.Hives are red and sometimes itchy bumps on your skin. An allergic reaction to
a drug or food usually causes them. Allergic reactions cause your body to release chemicals that can make your skin swell
up in hives. People who have other allergies are more likely to get hives than other people. Other causes include infections
and stress.Hives are very common. They usually go away on their own, but if you have a serious case, you might need medicine
or a shot. In rare cases, allergic reactions can cause a dangerous swelling in your airways, making it hard to breathe - which

face that is often misdiagnosed as adult acne. It varies in severity and does not always worsen with time. Typically,
you will experience inappropriate flushing that is not usually associated with sweating and/or persistent facial redness.
It is common to have broken blood vessels (telangiectasias) on your cheeks. You may also experience bouts of inflammation
that cause red papules (small bumps) or pustules. However, comedones (blackheads and whiteheads) are not part of rosacea.
About 50% of those with rosacea suffer eye involvement, including such conditions as conjunctivitis, blepharitis, dry gritty
eyes, and recurrent sties. Nose enlargement (rhinophyma) is uncommon and is mostly seen in men.Do I Have Rosacea?is a
chronic progressive skin condition that causes inflammation and swelling of the face. As the disease progresses,

Skin Tags: Should They Be Removed?
Skin tags are one such skin abnormality, and they are also very common. Skin tags - small portions of skin
that appear to be attached to or protrude from normal skin - are a common benign condition of the skin. Usually just a few
millimeters in diameter, skin tags vary in appearance. Some are the same color as surrounding skin while others are hyperpigmented
(darker than surrounding skin). In most cases, they are attached to the underlying skin by a small band of tissue called a
stalk . The medical term for skin tag is acrochordon. Other synonyms that have sometimes been used to refer to
skin tags include soft warts, soft fibromas, fibroepithelial polyps (FEP), fibroma pendulans, and pedunculated fibroma. Skin tags can occur in anyone
but tend to increase in frequency as we age. They are also more common in obese persons, possibly due to increased irritation and rubbing of skin folds. Hormonal factors or aging processes may also play
a role in the development of skin tags. Skin tags are most commonly found in or near the armpits (axillae) and on the neck,
but they occur commonly in all skin folds (such as the groin) and on the back and abdomen. Most skin tags are asymptomatic, meaning that they cause no symptoms. They are almost universally benign; reports of malignancies arising in skin tags are
extremely rare. Skin tags can become irritated and inflamed, especially when located in skin folds or in areas where clothing
rubs against them. Most dermatologists recommend removal of skin tags only when they are irritated or a source of
discomfort, or if they constitute a cosmetic problem. Skin tags can be easily removed in Dr. Rothfelds' office

V-Beam Laser Treatment
For Face Veins, Rosacea
and other Vascular Lesions
The V-Beam laser is a pulsed dye laser that can
successfully treat: - Face veins (telangiectasias)
- Facial,
neck and chest redness
- Rosacea
- Hemangiomas (large and small red bumps or growths)
- Red birthmarks (port wine stains)
- Other vascular lesions, including venous lakes (blue spots,
usually found on lips)
- Scars
The
V-Beam produces an intense but gentle burst of light that selectively destroys the blood vessels within the treated area,
without damaging the surrounding tissue. Red veins respond better than blue veins and smaller vessels respond sooner than
larger ones. While one treatment will produce some improvement, two to four treatments will probably be needed to achieve
the best results. Treatments are usually performed four weeks apart. Do not apply any anesthetic (numbing) gels.
Treatments are well tolerated without them. The feeling has been described as being snapped by a rubber band or as a slight
stinging sensation. Following treatment the area may feel warm or sting slightly like a sunburn. Improvement
can usually be seen immediately. Immediately following the treatment however the area may appear red or bruised and possibly
slightly swollen. Swelling usually resolves within 1 – 5 days while bruising or redness may last 7-14 days. Crusting
can rarely develop when port wine stains (red birthmarks) are treated. As with many lasers, color changes (darkening or lightening
of skin color) can occasionally occur in darker skin types as can scarring (in less than 1% of cases.) For important
social events, treatment should not be done within 14 days before the event.

Chemical Peel What is a
chemical peel? Chemical peeling uses a chemical solution to improve the skin's appearance. It can reduce or eliminate fine lines under the eyes and around the mouth, correct uneven skin pigmentation, remove precancerous skin growths, and soften acne or treat scars caused by acne. The procedure
can also treat wrinkles caused by sun damage and scarring, as well as skin blemishes common with age and heredity. Chemical
peels can be performed on the face, neck, chest, hands, arms, and legs. Possible
complications associated with chemical peels: Possible complications associated with
chemical peels may include but are not limited to the following: - change in skin tone color
For certain
skin types, there is a risk of developing a temporary or permanent skin color change. Taking birth control pills, being pregnant,
or having a family history of brownish discoloration on the face may increase the possibility of developing the abnormal pigmentation.
- scarring
Chemical peels can cause scarring. However, if scarring
occurs, it can usually be treated effectively.
- cold
sores and fever blisters
Those who are susceptible to cold sores, or herpes simplex infections, may have a reactivation of cold sores
or fever blisters following a chemical peel. A chemical peel is most commonly performed for cosmetic reasons to enhance appearance and self-confidence and may
be performed in conjunction with a facelift. However, a chemical peel is not a substitute for a facelift and does not prevent
or slow the aging process. What substances
are used for chemical peels? Phenol, trichloroacetic acid (TCA), and
alphahydroxy acids (AHAs) are used for chemical peels. The precise formula used may be
adjusted for each patient. - alphahydroxy acids
(AHAs)
Alphahydroxy acids (AHAs) such as glycolic, lactic, or fruit acids are the
mildest of the peel formulas and produce light peels that can often provide smoother, brighter-looking skin. AHA peels may
be used to accomplish the following:
- reduce fine wrinkling
- treat areas of dryness
- reduce uneven pigmentation
- aid
in control of acne
- smooth rough, dry skin
- improve texture of sun-damaged skin
AHA peels may cause the following:
- stinging
- redness
- irritation
- crusting, flaking, or scaling
- dryness
Generally, no anesthesia is needed for AHA peels since they cause only
a slight stinging sensation during application. Protecting
skin from the sun is important following AHA peels. - trichloracetic acid (TCA)
Trichloroacetic acid (TCA)
can be used in many concentrations and is used to accomplish the following:
- smooth fine surface wrinkles
- remove superficial
blemishes
- correct pigment problems
TCA can be used on the neck or other body areas, and may require pretreatment
with Retin-A or AHA creams. This procedure is preferable for darker-skinned patients. Anesthesia
is not usually required for TCA peels because the chemical solution acts as an anesthetic. Although, sedation may be used
before and during the procedure to help the patient relax. Two or more TCA peels may be needed over several months to obtain
the desired result, although mild TCA peels may be repeated more frequently.
The results of a TCA peel are usually less dramatic than and not as long-lasting as those of a phenol
peel. More than one TCA peel may be needed to achieve the desired result. TCA-peel patients are advised to avoid sun exposure for several months. The procedure also may produce some unintended
color changes in the skin. - phenol
Phenol is the strongest of the chemical solutions and produces a deep peel. A phenol
peel is mainly used to accomplish the following:
- correct blotches caused by sun
exposure, birth control pills, or aging
- smooth out coarse
wrinkles
- remove precancerous growths
Phenol:
- should be used on the face only, as scarring may result if used on the neck or other
body areas.
- is not recommended for darker-skinned
individuals.
- may pose risk for patients with
heart problems.
- may permanently remove facial
freckles.
- may cause permanent skin lightening.
- may leave lines of demarcation.
Recovery
may be slow and complete healing may take several months. After
a phenol peel, new skin may lose its ability to produce pigment. The skin will be lighter
and will always have to be protected from the sun. About the
procedure: The procedure involves a chemical solution that is applied
to the skin. The solution causes a layer of skin to separate and peel off. The new, regenerated skin underneath is usually
smoother, less wrinkled, and more even in color than the old skin.

How Acne starts Acne affects almost everyone — more than 90% of all adolescents, nearly
50% of all adult women and 25% of all adults. Crossing gender lines as well as national borders, it's one of the most
widespread medical conditions in the world. Yet there's still no cure. But there is hope. While acne is not curable, it

A blemish begins approximately 2–3 weeks before it appears on your skin's surface. It starts in your sebaceous hair
follicles — the tiny holes commonly called pores. Deep within each follicle, your sebaceous glands are working to produce
sebum, the oil that keeps your skin moist and pliable. As your skin renews itself, the old cells die, mix with your skin's
natural oils, and are sloughed off. Under normal circumstances, these cells are shed gradually, making room for fresh new
skin. But sloughing is different for everyone. Some people shed cells evenly; some don't.
Uneven shedding causes dead cells to become sticky, clumping together to form a plug — much like a cork in a bottle.
This plug, or comedo, traps oil and bacteria inside the follicle. A plugged follicle
The plug traps oil and bacteria within the follicle, which begins to swell as your skin continues its normal oil production.
Your body then attacks the bacteria with a busy swarm of white blood cells. The whole process takes 2–3 weeks, culminating
in a pimple.
An inflamed acne lesion
Why me? There is no one simple "cause" of acne — the condition is influenced
by many factors, many which are out of your control. The regularity with which you shed skin cells can change throughout your
life. The rate at which you produce sebum is affected by your hormone balance, which is often in flux — especially for
women. Research has also shown that genetics play a big part in the development and persistence of acne, so your family history
is a valuable prediction tool as well when considering the various causes of acne. One of the best weapons in the fight against
acne, however, is knowledge; if you know what causes acne, it's easier to formulate a good plan
of attack. There are five primary culprits contributing to this process. Each of these factors may vary dramatically between
individuals. While you don't have control over these factors, understanding them can help you in your search for the proper
acne treatment. Acne Causes - Culprit #1: Hormones.
For the majority of acne sufferers, the trouble begins at puberty, when the body begins to produce hormones called androgens.
These hormones cause the sebaceous glands to enlarge, which is a natural part of the body's development. In acne sufferers,
however, the sebaceous glands are overstimulated by androgens, sometimes well into adulthood. Androgens are also responsible
for acne flare-ups associated with the menstrual cycle and, on occasion, pregnancy. Acne Causes - Culprit #2: Extra
sebum. When the sebaceous gland is stimulated by androgens, it produces extra sebum. In its journey up the follicle
toward the surface, the sebum mixes with common skin bacteria and dead skin cells that have been shed from the lining
of the follicle. While this process is normal, the presence of extra sebum in the follicle increases the chances of clogging
— and can cause acne. Acne Causes - Culprit #3: Follicle fallout.
Normally, dead cells within the follicle shed gradually and are expelled onto the skin’s surface. But in patients with
overactive sebaceous glands — and in nearly everyone during puberty — these cells are shed more rapidly. Mixed
with a surplus of sebum, the dead skin cells form a plug in the follicle, preventing the skin from finishing its natural process
of renewal. Acne Causes - Culprit #4: Bacteria. The bacterium
Propionibacterium acnes, (P. acnes for short) is a regular resident of all skin types; it’s part of the skin’s
natural sebum maintenance system. Once a follicle is plugged, however, P. acnes bacteria multiply rapidly, creating the chemical
reaction we know as inflammation in the follicle and surrounding skin. Acne Causes - Culprit #5: Inflammation.
When your body encounters unwanted bacteria, it sends an army of white blood cells to attack the intruders. This process is
called chemotaxis; or, simply put, the inflammatory response. This is what causes pimples to become red, swollen and painful.
The inflammatory response is different for everyone, but studies have shown that it is especially strong in adult women.
What can I do?
Fortunately, you have options! There are many kinds of acne treatments available today. But first, you should
try to determine the type and severity of your condition. Acne, like a person, is highly individual — it can take
many forms, and have a highly variable response to treatment. The more you know about your specific form of acne,
the more likely you are to find a treatment that works for you.
